OPT-OUT QUALIFIER

This program is designed to provide you with information concerning the intermediate opt-out option of the National Diet Drug Settlement Agreement.  If you accurately answer the following series of questions, this program will tell you whether you qualify as an intermediate opt-out.

For fastest response, complete the results questionnaire after completing the program and fax all echocardiogram results and copies of all forms filed with the Settlement Administrator, to the above fax number.  Include your name and phone number on all fax submissions, and we will respond.

PLEASE NOTE:  You are not eligible for an intermediate opt-out if you filed a pink form (Accelerated Implementation Option) or orange form (Initial Opt-Out) with the settlement administrator.
